I had "unobstructed coronary arteries" after coronary angiogram 4 yrs ago- does this remain or is there a chance of cholesterol building up over time?

It is unlikely that you will have developed significant coronary artery disease in the intervening 4 years however this is dependent on your personal risk factors. If you have no symptoms of angina or significant effort intolerance, and your cardiac risk factors are well managed, then it is unlikely further testing would be helpful at this point. If testing is required then the correct test would have to be determined by your cardiologist.
