Mammograms is still the best modalities for the breast , of cancer spread to other areas so CT scan and bone scan are the best for the rest of the body

This is a very general question. however, to diagnose recurrence of a breast cancer;
1.History of the patient; for example feeling a lump in the breast or armpit, back or groin pain, shortness of breath, loss of weight, extra
2. Proper examination by a breast specialist
3. investigation; Mammogram, ultrasound, and MRI for local recurrence in the breast and the armpit.
for the spread of cancer outside the breast and armpit, and only in the presence of symptoms, for example back or groin pain, shortness of breath. Then investigation like CT scan, bone scan, PET scan, MRI will be required

Most of them do not need anymore than routine screening with clinical examination and Mammogram and targetted Ultrasound

Majority of the patients do NOT need any further enhanced follow up either with CT scan Or Bone scan unless

1) New systemic symptoms - e.g. Bone pain, breathlessness, loss of weight or loss of appetite

2) Some patient with advanced breast cancer- multidiscplinary discussion may be required which may recommend initial above staging investigations with targetted screening for indeterminate lesions
